BEFORE YOU START, you will need:

USA Lacrosse Membership Number (ex: 000015289991). Every player must be a US Lacrosse Member for insurance purposes. To become a member, please go to: https://account.usalacrosse.com/login/
Payment Methods Accepted – Venmo/ApplePay/Other ($500/player). Please see Step 1 below for full payment instructions.
Multiple Players – for any families with multiple kids joining the team, please fill this form out for each athlete.
